I work at the CDCD (Cognitive Disorders and Dementia Center) in Modena for the AUSL company. I deal with the global care of people with cognitive disorders and their families, coordinating the hospital-territory pathway and social services. My work includes needs assessment, personalized care planning, health education for caregivers, and monitoring of therapeutic adherence. I work in a network with doctors, psychologists, social workers, and GPs to ensure continuity, reduce inappropriate access to services, and improve the patient's quality of life.
